Type of Position: Permanent Full-time

Shift/Work Schedule: Monday – Friday (7 AM – 5PM) with rotating telephone call

Work Hours: 8 hour shifts

Call Schedule: Telephone call only – no hospital rounds 

Start Date: ASAP

Facility Type: Clinic with 3 locations and 2 Ambulatory Centers 

Clinical Setting: Outpatient

PPD: 10-15 each provider (65-80 total)

Procedures required: Neurosurgery –  Pain injections, Rhizotomy. mild procedures, angiogram.

Support Staff (if applicable): 30 support staff 

Credentialing Time Frame: 30 days or less.

Location: Franklin, Kenosha and Lake Geneva 

Job Description / Responsibilities) Assist provider with clinic patients and procedures 

Credentialing and Licensing Requirements: PA – Certified 

Willing to accept new grad or Board Eligible candidate?: Yes but prefer experience 

Willing to wait for State Medical License? Yes but prefer candidates with active WI license.

Willing to accept J1 or H1 visa candidates?: No

Salary: $105,000 – $150,000 depending on experience
